History of the Microtransitions in Care Lab

Our Story

Our lab's work began in the Transitions of Care Subcommittee under the Clinical Practice Committee of the Post-Acute and Long-Term Care Medical Association (PALTMed).  The COVID-19 pandemic taught us how consequential trips and appointments could be for residents when visitation and outings were restricted to prevent infection spread. These trips can serve as a source of both meaning and risk. We coined the term "microtransition" to call attention to this important phenomenon. Our team continues to do work to improve the quality of microtransitions for all post-acute and long-term care residents.

Our Mission

It is our mission to improve care coordination, autonomy, and safety during room changes, recreation outings, and outpatient appointments through research, training, and dissemination. We strive to identify gaps in communication during these microtransitions and work to develop practical tools for clinicians and caregivers to improve these transitions in custodial care.
