Sing Palat, MD, CMD

Dr. Sing Palat is an Associate Clinical Professor of Medicine in the Division of Geriatric Medicine at the University of Colorado, Medical Director with Optum Health’s Senior Community Care program, and a Co-Investigator in the Microtransitions in care Lab. She completed her medical training at the University of Pittsburgh School of Medicine, including residency in internal medicine and fellowship in geriatric medicine, and is a Certified Medical Director (CMD). She currently serves as the President of the Colorado Society for Post-Acute and Long-Term Care Medicine and previously served as Chair of the Post-Acute and Long-Term Care Medical Association (PALTmed) Transitions of Care Subcommittee and Secretary on the PALTmed Board of Directors.
Throughout her career, Dr. Palat has led interdisciplinary initiatives to improve safety and quality in long-term care, including projects to reduce avoidable hospitalizations, optimize antibiotic and diagnostic stewardship, and strengthen communication across transitions of care. Her research and leadership helped define the concept of microtransitions—the small but consequential movements between rooms, clinics, or care environments that shape residents’ safety and autonomy. Through her previous leadership roles at PALTmed, she has organized national symposia and educational programs that bridge research, policy, and front-line practice, and facilitated the development of the Microtransitions Best Practice Recommendations Consensus Statement.
Dr. Palat's current role in the Microtransitions in Care Lab focuses on serving as a liaison with organization partners such as PALTmed, and development of the Lab's tools for clinicians.
Fun Fact: Dr. Palat enjoys playing the piano in her spare time.
